Physician Signature Form
A letter from your child's physician stating they are ok to attend daycare can be used in place of this form.

On-Site Field Trip Permission Slip
The CDC does not have field trips off campus. This form is permission for your child to view and/or touch Fire Trucks, Police Cars, Ambulances, potentially animals (all parents will be contacted before any animals are allowed to visit the CDC), etc. on the blacktop behind the school.
